But as the trend grows, questions about equity, access, and whether the premium cost delivers genuinely better outcomes deserve examination.
In a typical concierge arrangement, patients pay an annual retainer fee, generally ranging from $2,000 to $15,000 in Palm Beach County, in addition to their regular insurance. In exchange, they receive enhanced access including same-day or next-day appointments, extended visits (30 to 60 minutes versus the standard 15), direct physician communication via phone or text, and comprehensive annual wellness evaluations.
The Patient Experience
Proponents point to dramatically reduced wait times and the ability to build a deeper physician-patient relationship. Dr. James Chen, who transitioned his Wellington practice to a concierge model three years ago, reduced his patient panel from 2,500 to 600. "I can actually practice medicine the way I was trained to," he said. "I spend time with patients, address their concerns thoroughly, and catch problems early because I'm not rushing through 30 patients a day."
Patients who have made the switch frequently cite the peace of mind that comes with knowing they can reach their physician directly. For affluent retirees managing multiple chronic conditions, the model can be genuinely valuable.
The Equity Question
Critics note that every physician who transitions to concierge medicine removes themselves from the general patient pool, exacerbating primary care shortages for the broader community. With Palm Beach County already facing a projected deficit of 120 primary care physicians by 2028, the trend raises legitimate concerns about healthcare access for those who cannot afford membership fees.
Some practices have attempted to address this by maintaining a portion of traditional patients alongside their concierge panel, though this hybrid model can create a two-tiered experience within the same office.
